As a Life Care Planning Specialist I am an advocate for the future of the patient. It is my role to summarize the medical, educational, vocational, psychosocial, and daily living needs of the individual who can function only with professional assistance. I will project long term costs of care, and establish rehabilitative goals while coordinating future care providers in order to best assure a continued recovery.

The definition of A Life Care Plan is a dynamic document based upon published standards of practice, comprehensive assessment, data analysis, and research, which provides an organized, concise plan for current and future needs with associated costs for individuals who have experienced catastrophic injury or have chronic health care needs.
